Campus Survey Panel Approval
In order to coordinate surveys at the University of Mississippi we ask that you complete the Survey Panel Group (SPG) application form. Before completing this survey distribution panel application, please review the SPG policies and have the appropriate information available. Please refer to Administration of University of Mississippi Survey Distribution Panels found here.
In this form, you will be asked to provide the following information:
- Title of your survey
- Purpose of your survey
- Your contact information
- Approved recruitment email message(s) (Word or PDF)
- Description of distribution panel member characteristics (i.e., characteristics of individuals who will receive survey) such as campus location, type of student/faculty/staff
- Proposed date for initial survey recruitment and follow-up emails
- Copy of IRB approval, if applicable
Requests for sample panels for surveys from individuals or organizations outside of the University will rarely be approved. However, an electronic copy of the UM directory can be purchased from the Office of the Registrar. Please send request to registrar@olemiss.edu.
