2025 Community Investment Program
Mayo Clinic Health System is committed to giving back to the community. All fund-seekers are asked to provide answers to required questions (as noted with a an *).
A current W-9 is required where the name and address match the organization and address in the application. You may also attach additional information about your application at the end of the form.
There are two categories of community investment.
- Sponsorships are typically for events where there will be visibility for sponsors. Sponsorship applicants are asked to answer the required questions at a minimum.
- Contributions provide funds for programs or projects that have impact in the community. Contribution applicants are asked to answer all the questions on the form to the fullest extent possible and attach a project budget outlining funds and expenses.
Please note that requests over $1,000 will be reviewed quarterly (February, May, August, November), and must be submitted by the first day of the month for review. Requests $1,000 and under will be reviewed bi-weekly.
