REDCap ToxIC Data File Request - Preliminary or Site Only Data

ToxIC data files for research will be provided upon request to investigators at active participating sites. Data exports will include a standard set of fields as documented at toxicregistry.org/members only under "ToxIC Data File Requests." Any additional fields can be requested in the last form field. Please review the ToxIC Data Form to identify the available fields.

ToxIC Investigators are required to abide by the terms of the ToxIC Data User Agreement (DUA) - available on the Members Only page at ToxicRegistry.org. Please check off the box to indicate your agreement to the terms.

Basic Inclusion Criteria - Used to define/limit the Cases Eligible for Data Set

Indicate data reporting years required, age range and sex, etc. Defaults are set as indicated.

In the sections below please select any major clinical selection criteria needed to define the cases contained within your dataset. Defaults are set as indicated. Indicate any clinical sign(s) to LIMIT the data set - Enter Section below with specific sign(s) in the next field. Example 1: Limit to single inclusion criteria within a section. Data request to cases reporting a seizure Clinical Sign (Section): Enter 'Nervous System' Clinical Sign Section Detail - Type in 'Seizure' Example 2: Limit to >1 criteria within a section. Data request to cases reporting a seizure and/or coma. Clinical Sign (Section): Enter 'Nervous System' Clinical Sign Section Detail - Type in 'Seizures' and 'Coma' Example 3: Limit to 1 or more criteria within a >1 section. Data request to cases reporting a seizures and ventricular dysrhythmias Clinical Sign (Section): Enter 'Nervous System' Clinical Sign Section Detail - Type 'Seizures' and 'Cardiovascular/Ventricular dysrhythmias'

For example - Select ONLY cases with any "Cardiovascular Clinical Sign" then select "Cardiovascular"; OR, If require >1, for example "Cardiovascular" and "Renal/Muscle" then type second clinical sign section in detail text box below:

For example - Select ONLY cases indicating "Ventricular dysrhythmias"

Clinical Inclusion Criteria - Labs (Section)

Select the lab data (REDCap mandatory field) below if part of the inclusion criteria (Yes/No, Done/Not Done, etc.) as indicated on the REDCap Core Registry data items.

For example: If only interested in cases with EKG values available, then enter - Only include cases if EKG="Yes" and Initial EKG Data = "Known"

Clinical Inclusion Criteria - Treatment

For example - Select ONLY cases indicated as receiving any "Antidote" then select "Antidote"; OR, If interested in just a specific type of antidote, then select "Antidote" and indicate specific antidotes (as listed on REDCap data form) below; OR. If require >1, for example "Antidote" and "Pharmcological Supoort" then type second treatment section below in detail text box.

For example - Select ONLY cases indicating "Sodium Bicarbonate" [Antidote] and "Vasospressors" [Pharmacologic Support]

Standard Data Fields Included in Data Deliveries:

All standard reports include the following: ToxIC ID# Basic Demographics: Age Category, Specific Age (Years or Months), Race, Hispanic Ethnicity, Sex Encounter/Exposure: Reason for Encounter (Primary & Secondary) Did the Patient Have a Toxicological Exposure? Any Class/Agent fields in cases meeting inclusion criteria (blank fields/columns will be removed) Clinical Information: Did the Patient Have Signs & Symptoms If Yes, are Signs & Symptoms Tox Related? Treatment Information: Was Toxicological Treatment Given? If Yes, the following Tox Treatment Sections are Standard Antidotes, Pharmacologic Support, Decontamination, Elimination, Non Pharmacologic Support Please review the detail data collection form on REDCap Core Registry File Repository and/or Members Only section at ToxicRegiistry.org and complete the sections below regarding any additional fields to include in the data file.
