This form is necessary to obtain special privileges in SiteManager for the purpose of entering information in sample records and data in material test templates on behalf of a Certified Technician.

This form is a supplement to the “SiteManager UserID Add/Change Form”.

A Certified Technician is defined as someone who holds a current certification issued by the Oklahoma Highway Construction Materials Technician Certification Board.

The status of Technician Certifications is maintained in SiteManager. This data allows or restricts access to certain areas of SiteManager based on the user’s certification status.

The restriction of privileges based on certification status serves the intent of an electronic technician certification seal of a sampler on a sample record, and of a tester in the material test templates.

SiteManager Users who complete this form and receive privileges to enter material data on behalf of a certified technician agree to strictly follow additional requirements provided below.

Failure to follow the requirements shall result in revocation of SiteManager privileges.

In addition to following the Department’s Policies and Procedures, Quick Reference Guides and SiteManager training, concerning the proper documentation of sampling/testing activities, the following additional instructions are required to be followed when completing a SiteManager sample record of a material governed by the Certification Board on behalf of a Certified Technician:

Paperwork used to enter laboratory test results must come from a Qualified Lab.

Paperwork used to enter sample information and test results must be stamped by the Certified Technician(s) showing who sampled and tested the material.

Sampler and Tester(s) must be entered in the correct SiteManager fields by selecting from the filtered pick list of Certified Technicians.

If the Certified Technician(s) are not available for selection, the sample record must not be completed and cannot be authorized for the purpose of satisfying S&T requirements on a contract.

I will take all reasonable precautions to protect my AASHTOWare Project SiteManager password.

I will not give my AASHTOWare Project SiteManager password to anyone else or allow anyone else to use AASHTOWare Project SiteManager under my account.

If I have reason to believe that my AASHTOWare Project SiteManager password has been compromised, I will notify my supervisor and change the password immediately.

I understand that my account can be revoked and/or future contracts with ODOT may be jeopardized, if I fail to properly secure my password.

I acknowledge that any data entered into AASHTOWare Project SiteManager becomes the primary source documentation for a construction project.

I understand that to maintain a proper audit trail, AASHTOWare Project SiteManager tracks every entry made in the system by user ID.

Whenever I enter any information into AASHTOWare Project SiteManager, it will be treated as though I had written it on paper and signed it.

I understand that all data entered into AASHTOWare Project SiteManager under my password will be used for the purpose of documenting the work performed on a construction contract and used to pay a contractor.

I also acknowledge that the information entered in AASHTOWare Project SiteManager is, to the best of my knowledge and belief, true and correct and is equivalent to signing the following statement:

“The item(s) and material(s) were inspected and found to conform reasonably with the plans and specifications except as noted.”
