OPT Reporting Form


While on Optional Practical Training (OPT), a student is required to report the following to SEVIS:


• Change in student’s legal name.

• Change in student’s address while on OPT.

• Changes in employment or interruption of employment


Students should have access to their own SEVIS OPT portal, and we encourage you to submit your own updates. This form acts as an important backup so that ISSS can help in case there are any issues with the portal.


Form submission reasons:

New employer = report a new job

Job ended = report an end date for your most recent job

Contact update = report any changes in your contact information (address, email, phone number, legal name)
