One Minneapolis Fund Application 2018

You must upload each attachment one at a time.

- Detailed project schedule with work plan.
- Key staff with relevant qualifications (2 pages maximum)
- List of board members and their affiliations.

- Most recent year's financial statements with a balance sheet.
- Most recent year's organizational budget (highlight income from government)
- Project budget

- Most recent 990 tax form or audit.
- Current IRS 501(c)(3) determination letter (or fiscal agent's letter).
- Proof of general liability insurance with $2 million in coverage.
- Annual report (if available).
