NMT Chemical Inventory Submission Form

This form is for PI's, Lab Managers, and other responsible parties to submit their Chemical Inventories to Research Compliance & Safety.

 
 
 
 
 
 
 

Upload your chemical inventory files here. Research Compliance & Safety recommend that your chemical inventory document have columns for the following items:


  • Chemical Name: Full name as listed on the Safety Data Sheet (SDS); avoid abbreviations.
  • CAS Number: Unique Chemical Abstracts Service number to avoid ambiguity.
  • Manufacturer: Company that produced the chemical.
  • Product Name/Catalog Number: As labeled by the manufacturer (helpful for reordering or looking up the SDS).
  • Physical State: Solid, liquid, gas.
  • Quantity on Hand: Amount present, with units (e.g., 500 mL, 2 kg).
  • Container Size: Size of individual containers (e.g., 100 mL bottles).
  • Number of Containers: Useful for bulk storage tracking.
  • Location: Building, room, and specific storage location (e.g., flammables cabinet).
  • Date Received: Helps with shelf-life tracking.
  • Expiration Date: Especially important for unstable or reactive chemicals (e.g., ethers, peroxides).
  • Hazard Classification: GHS pictograms or brief categories (e.g., flammable, corrosive, toxic).
  • SDS Available: Yes/No or link to digital SDS.
  • Owner/Responsible Person: Person or lab responsible for the chemical. Often, this is the PI of the lab.
  • Disposal Status: Notes on whether the chemical is active, waste, or scheduled for disposal.
