General Information

This Safety Prequalification Form is for Qualico Developments West Ltd. and the following Qualico Business Units: Broadview Homes, Sterling Homes, NuVista Homes, Augusta Fine Homes, Qualico Communities, Qualico Properties, Star Building Materials and StreetSide Developments.


If you work with more than one Qualico Business Unit, you may receive more than one invitation to fill out this form. However, please note that you will only be required to fill this form out once per year.


Once you have been awarded a scope of work, you must complete the online Safety Prequalification and acknowledge that you will comply with Qualico Developments West Ltd. Safety Policy's. Completion of the online Safety Questionnaire and compliance with the Qualico Developments West Ltd. Safety Policy's throughout the performance of the work is a condition of doing business with Qualico Developments West Ltd. Any contract may be terminated with cause for failure to complete the online Safety Questionnaire or failure to comply with Qualico Developments West Ltd. Safety Policy's at any time during the performance of the scope of work. Completion of the Safety Prequalification Form by your company must be done yearly. Ideally, it should be completed in December of each year for the upcoming calendar year.


Should you work for Streetside Developments on Multi-Family Construction Sites, as the employer, you are responsible to ensure your workers and any subcontractors working on your behalf have completed the online Streetside site-specific safety orientation prior to them starting work at the job site. The workers must connect with the Site Supervisor prior to starting work at the site for the first time to receive their hardhat sticker. Workers will be required to use the electronic QR code for the daily sign-in / sign-out procedure.

It is important to note that any and all required documentation must be presented when requested by a Qualico Developments West Ltd.’s representative, Construction Safety Officer and or an Occupational Health and Safety Officer.


Designated washroom facilities must be used on-site. Any individual caught urinating or defecating on site and not using the designated washroom facilities will be removed and banned from all Qualico Developments West Ltd. sites.


To ensure a safe and productive work environment, smoking (including vaping and cannabis products) is prohibited on the Work Site. It is essential that all workers remain fit for duty, as the consumption of any substances that may impair their abilities can compromise safety and efficiency on construction sites.


Any Ground Disturbance work should be completed by trained individuals and proof of locates acquired prior to work starting. Locates should be available at all times on site as well. It is the responsibility of the contractor to acquire locates for the work they are tasked with.


Contractors are strictly prohibited from disconnecting, modifying, relocating, or tampering with any temporary fuel heater devices provided by Qualico. If relocation of the temporary fuel heaters is necessary, the Home Builder Site Superintendent must be contacted.


All work sites must be fenced, and the fencing maintained in accordance with the Alberta Building Code. When any member of the contractor group is the last person to leave a work site, the fencing at the work site must be closed and secured by that person. If unable to do so; the Site Supervisor for the site should be contacted immediately. Failure to secure the site at the end of the workday may result in the contractor being held responsible for any fines or prosecutions levied against Qualico.


Qualico Developments West Ltd. reserves the right to implement “Stop Work Orders” for those contractors and sub-contractors who are in non-compliance with Qualico Developments West Ltd.’s own Health and Safety Management Principals. This includes non-compliance with Alberta government Occupational Health and Safety legislation, and any Alberta cities by-laws.

Safety Rules

  • Incidents, injuries or “near misses” regardless of the nature will be reported immediately to the Site Supervisor/Area Construction Manager. The contractor will analyze (investigate) all these events and provide Qualico with a copy of the report.


  • A hazard assessment will be completed for each job before work starts. For long-term jobs, the hazard assessment should be reviewed at least daily or revised when conditions change.


  • Wear all required PPE, as per the Minimum PPE Requirements below and your hazard assessment.


  • All tools, equipment and PPE will be inspected before use and will be used only as intended and designed by the manufacturer. The removal of guards or safety devices is strictly prohibited.


  • Heavy equipment, such as telehandlers and AWP should be inspected and maintained as per the manufacturer's specifications and proof of maintenance and/or inspection should be available on-site or affixed on the equipment.


  • All trades operating tools or equipment should be competent (properly trained and with enough experience) in the operation of said tools and equipment.


  • All electrical hand tools shall be ground or double insulated.


  • Lock-out/Tag-out and remove from use all defective tools and equipment.


  • No person shall ride on or work from platforms that are not designed for people (such as working from a stack of OSB on the forks of a telehandler).


  • All young and un-experienced workers will be under the continuous supervision of an experienced worker. Young workers (16 to 18 years old) will only perform low-hazard work, such as cleaning). No workers under the age of 16 shall enter active construction sites.


  • Clean as you go and don't allow debris and materials to accumulate and hinder access and egress or create tripping hazards.


  • Avoid running on construction sites, in the case of an emergency please evacuate quickly but safely.


  • All work shall be carried out in accordance with appropriate safe work practices, job procedures and the site supervisor’s direction.


  • No open flame heaters allowed on site at any time. Fire extinguisher must be in the direct vicinity of any contractor provided heaters. Fuel containers must be kept outside of any wood structure at all times. Refueling must be performed outside of any unit under construction.


  • Smoking/Vaping is not permitted on the worksite.


  • Possession or use of weapons, firearms, intoxicating beverages or prescribed and un-prescribed drugs with potentially impairing effects is strictly prohibited while on the job.


  • Fall protection is mandatory when working at heights greater than 3M/10ft or when fall hazards are present. Workers may be asked to produce proof of fall protection training/competency.


  • Horseplay or fighting will not be tolerated.


  • Violence and harassment in the workplace is strictly forbidden and will be investigated. Disciplinary action may include termination of contract or permanent site eviction of individuals involved.


  • Site fencing (when present) must be closed and secured when leaving as per Alberta Building Code.


Minimum PPE Requirements (Active Construction)

The following are Qualico West Developments Ltd. minimum PPE requirements for Qualico employees and contractors:


  • All Qualico personnel, including supervisors, sales, the management or health and safety, will wear, at a minimum, a hard hat, high-visibility safety vest and safety boots.


  • Contractors, up to and including the framing stage will wear at minimum a hard hat and safety boots. (High Visibility in the presence of heavy equipment)


  • After the floors are installed, indoor trades can wear regular shoes, unless there are other foot injury hazards.


  • Please note that additional PPE might be required depending on the task at hand and hazards present on-site (such as fall protection, respirators, etc).


*Prime contractors will follow and enforce their own PPE policy.


Safety Training Requirements

Below are the required safety training for our contractors, based on the work performed:


  • Work at heights above 3m/10ft or when fall hazards are present requires individuals to have completed Fall Protection (End User) training.


  • Operation of heavy equipment including skid-steer, telehandler and zoom-boom requires individuals to have completed operator training.


Emergency Equipment and Personnel

Below are the minimum requirements for emergency supplies to be available on-site at all times:


  • First aid kit (Alberta #2)
  • Fire extinguisher (ABC, min 5 lbs).
  • At least one Certified First Aider.
  • Spill Kit (If operating heavy equipment)
