Request for Return Goods Authorization
This form is required to be filled out and submitted BEFORE an RGA is provided. THIS IS ONLY AN AUTHORIZATION REQUEST. Please DO NOT ship any items until ZEPHYR provides you with an RGA number.
Once you receive an RGA number you may ship your items. The items MUST be received and evaluated by Zephyr to verify the request. No credit will be provided until the items are received and evaluated. Returns are subject to a 25% restocking fee. Zephyr reserves the right to waive this fee with an offsetting order at the discretion of the company.
DO NOT SEND ADDITIONAL ITEMS AND QUANTITIES THAT ARE NOT ON THIS REQUEST.
RGA numbers can only be used ONCE and cannot be duplicated. DO NOT SEND ITEMS BACK USING A PREVIOUSLY ASSIGNED RGA NUMBER.
