Per recent revisions to APM-710, academic appointees who don't accrue sick leave have a new bank of six days per calendar year, distinct from your existing paid medical leave options, to be used at your discretion.

The purpose of this form is to give you a quick way to report your use of that leave, without needing to know who to tell. We'll just need a few basic details.
