Research Gift Intake Form

Welcome to University Development and Alumni Relation's (UDAR) Research Gift Intake Form. This form has been designed for research administrators, financial analysts, and advancement staff to submit the documentation and information necessary for UDAR to accept and process gifts that support research. Please use this form if one of the following applies:


  1. To request a research gift agreement​.
  2. An agreement or letter for a research gift needs to be reviewed and signed by an authorized signatory.*
  3. A new check or wire for a research gift has been received, or will be arriving shortly, and needs to be processed and deposited into an existing fund or a new fund.


Through this form, we will request the information we need to ensure that the transaction you have can be accepted by UC Berkeley as a gift and will comply with relevant University policies and guidelines, as well as IRS rules.


*UDAR is the only unit on campus who can accept and sign documentation for gifts on behalf of UC Berkeley.

The PI is the individual who has primary responsibility for the design, execution, and management of a research project and who will be involved in the project in a significant manner.

Many corporate donors will provide their own gift letters. If the donor is open to receiving and signing a UC Berkeley research gift agreement instead, we are happy to draft one for your review.

Provide pertinent information to help us connect the gift, documentation, and fund allocation.

Example: To support Dr. John Flanagan's research on the capability of manipulating biomechanical stretch and ischemia in the glaucoma research lab.

Gifts to a specific research project must be allocated to a fund that specifies support for that same research project. A gift cannot be added to a fund that has a more general purpose than that outlined in the documentation for the gift.

A Dept ID is required to establish a new research gift fund.

Be prepared to upload all that apply:

  • Gift/Award Letter - This could be an externally generated letter, email notification of award, or other email or document stating that a gift is being made.
  • Completed proposal or application for funding - This could be a completed application form requesting funding, a formal or informal proposal that was submitted to the funder, or communication with the funder discussing the nature of the gift.
  • Call for applications - This is the funder's call for nominations or call for proposals. Please include the funder's submission guidelines.
  • Signed 700-U Form - The only accepted electronic forms are those completed through the Phoebe/DocuSign integration. 700-Us initiated via the Phoebe/DocuSign integration with the gift checkbox checked, will automatically be sent to Gift Services and do not need to be uploaded via this form. Positive disclosures should come with the COI determination notification and related backup.
