This form should be filled out for events where all three of the following are true:

1. Organizers plan to serve alcoholic beverages.
2. University Catering is not serving the alcohol.
3. The event is hosted on UO owned, leased property, and for UO sponsored events off premises.

This form is not required if:

• The event is being hosted at restaurant and the restaurant  is providing service and food.
• This is for non-UO hosted event (weddings, etc.) and a special event permit is not required. https://www.oregon.gov/olcc/lic/Pages/special-event-licensing.aspx

NOTE: Please submit this form at least 14 business days prior to an event.

Documents Required for Approval:

1. An approved UO Catering waiver, if applicable.
2. A copy of the catering menu or food order.
3. A certificate of insurance from the vendor providing food service (if not using UO catering).
4. A certificate of insurance that indicates host liquor liability and names the University as additional insured from the vendor providing and serving alcohol.

Note: Private events held by members of the public do not need to complete this form and should work with UO event services directly.

UNIVERSITY CATERING INFORMATION

University Catering is the authorized caterer for events scheduled in University campus facilities. This includes the service of alcohol. Use of any other catering vendor must be approved by University Catering. To get approval, you must submit a request for a catering waiver to UO Catering at least 15 days prior to the event.
