This form is to be used to request statistical service by CIDA only for unfunded investigator-initiated projects. The cost of your project will be paid 50% Section funds and 50% department funds.

Your request will be routed first to your Section Head for approval. Then to the Vice Chair of Research for final approval.

When approved, the department will submit a request to CIDA. An assigned statistician will contact you about your project.
