2024 2025 Cell Phone Stipend Request Form
Department/ campus secretaries will submit a form for each employee who is to receive the cell phone stipend. A signed form is a requirement to submit including the employee, principal and director/supervisor signature.
To be included on the monthly pay, the form must be submitted by the 1st of each month. If the form is received after the first, the stipend will be issued the following month. [ Example, the form is submitted on October 10th, the stipend would not reflect until the November paycheck.] Retroactive payments will be issued if necessary and approved by HR.
If an employee has a district-issued cell phone, they are ineligible for the cellphone stipend.
