Small Business Enhancement Program Eligibility and Beneficiary Information Form

The City of Cambridge funds the Small Business Enhancement (SBE) Program with Community Development Block Grant funding from the U.S. Housing and Urban Development Department (HUD). HUD requires that we obtain the following information from all applicants to the Program. This information is used by the City to determine the eligibility of the applicant under HUD guidelines and is not shared with an outside party. You must be HUD-eligible in order to participate in the Small Business Enhancement Program. Please complete all applicable spaces on this document and be sure to sign. Incomplete Eligibility Forms will not be reviewed. This Program is first come, first served. There is no guarantee that you will receive a grant this fiscal year. Application Deadline: March 15, 2025. The deadline is final date that you can complete a full application. This Eligibility Form is only for SBE Part 1. If you are eligible, in SBE Part 2 you will submit your full application.”
