Please place test supply requests for one organization at a time. Items should be requested by the unit (e.g., 10,000 swabs) rather than by the case, as case size varies. All Abbott COVID-19 testing supplies are available as a kit and not individual components. To request Abbott COVID-19 testing supplies, please enter the number of kits you would like to receive.


If you are not requesting a particular item, please leave the field blank. The fulfillment team will review and fill your requests.


Supplies are not guaranteed. Orders will be fulfilled as long as supplies last.


NOTE: This form is only for Abbott COVID-19 Testing supplies. There is no option to request Self-Test kits on this form.

K-12 School Requests

K-12 schools can request BinaxNOW test kits through this online order form. For questions or more information about COVID testing supplies for K-12 schools, please contact Jamekaa Baker-Flowers at jamekaa.baker-flowers@dhsoha.state.or.us.

If you are requesting BinaxNOW for outbreak testing, please ensure that your Local Public Health Authority has been informed of your outbreak status.

Abbott ID NOW and Binax NOW testing kits are only approved for use in clinical settings, and organizations must have a CLIA number in order to receive these kits. Please enter your CLIA number here.
