2024-2026 CE Repeat Use Form

Application Overview

This application is valid for one of more of the same program to repeat if submitted between October 1, 2024, and September 30, 2026. The original application must have been submitted during the same time frame with an authorization number starting with B26- .

Applications must be submitted 3 days before the event.

Repeat application must have:

the same content, objectives, and instructional hours as those of the original program

the same presenter(s) or presenter(s) with comparable relevant experience and degree(s).

a comparable reading list for distribution. Changes and/or additional references are acceptable if for the purpose of improving and/or updating.

Application fee

There is a $15 charge for each session date that a program is repeated. Please submit a separate form for each session date.

Applications received less than 3 days before the event will require a rush processing fee payment of $75.

Payments must be received before an application is reviewed. Please submit payment here: https://www.bu.edu/ssw/careers/pep/payment/.

Please note: You must submit the application with all of the required information and attachments. You will not be able to submit or save partial information for later completion. If necessary, our team will email you to revise parts of the application to meet requirements and standards.
