Request Form for Minds of the Future Programming

Thank you for your interest in the programs offered by the Shock Trauma Center for Injury Prevention and Policy! Our goal is to provide engaging and research-validated educational programming for youth and student groups. Our MINDS OF THE FUTURE program is an immersive and hands-on experience offered on-site at Shock Trauma.


Applications are approved on a case-by-case basis depending on resources and availability.

Participants must be at least 16 years old. Max number of participants allowed is 24.

For every 10 participants, there must be at least one adult to chaperone the group. Max number of teachers/staff/chaperones allowed is 4.

Date/Time Information

This program is offered on weekdays only. If possible, please provide at least 3+ options, as well as any additional information that may assist in accommodating your request (i.e. "anything in June" or "nothing on Fridays").

Please note, 9am is the earliest arrival time available.

Please note, 2pm is the latest departure time available.

Preferred Experience(s)

We recommend incorporating at least two (2) activities/experiences into your program. For further information regarding each of the below programs (such as content areas covered and their associated time allotment), please read our flyer.
