FOA Rally Celebrate Life Donor Form

OUR RALLY IS VIRTUAL THIS YEAR DUE TO THESE CHALLENGING TIMES! For the safety of our community during this challenging time, our signature FOA Rally 4 Recovery is VIRTUAL this year. This event will be packed with information about local and national treatment options, recovery supports, and stories filled with hope! Our email list contains over 2,500 people and we have over 11,500 Facebook followers. This will be THE virtual Rally event of the year! You can celebrate the life of your loved ones or friends who are: IN MEMORY donation - gone but never forgotten IN HONOR donation - progress made in their recovery journey The names and information will be added to a looped TRIBUTE SCROLL which will appear at the bottom of the live stream event. Live streaming can be viewed on our Facebook page, YouTube Channel, and our website on August 30, 2020, from 3 to 5 pm.
 

SPECIAL VIRTUAL FULL-PAGE TRIBUTE OPPORTUNITY

For tribute donations of $200 or more, in addition to looping, a full-screen image with text will be displayed once during the virtual event and emailed to you for a keepsake. Please be prepared to upload a good quality image of your loved one at the bottom of this form.
