Application for the Phlebotomy Technician Learnership at the PathCare Academy for 2026.

INSTRUCTIONS TO APPLICANT:

Information you provide in this application must be accurate and true.

Ensure that you meet the specific requirements i.e.

completed and passed NSC grade 12 and in possession of a matric certificate reflecting Life Science/Biology ≥ 50% WITH Mathematics ≥ 40% or Math Literacy ≥ 60%.

OR

completed and passed NCV Level 4 Primary Health and in possession of a certificate reflecting HBM ≥ 60% WITH Mathematics ≥ 40% or Math Literacy ≥ 60%.

Answer the questions honestly and only select one answer for the drop down questions. If a question does not apply to you then select the "Not applicable to me" option.

If you cannot provide proof in the form of a Matric or NCV certificate (documents will be verified) that you meet the requirements above DO NOT submit an application as it cannot/will not be considered. You will be required to upload a copy of your Matric or NCV certificate with this application. Applicants who meet the specific requirements and have followed instructions will be contacted via sms to participate in a rigorous recruitment process in the coming months.
