Please have the following documents ready to upload:

1. Business License
2. HVAC License (If applying to work on Heat Pumps)
3. Recent W9 (Must be March 2024 Revision)
4. IRS Link to Current W9 Form
5. Insurance Paperwork

• General Liability and Automobile Liability
• Minimum of $1M per occurrence with Raypak, Inc., 2151 Eastman Ave, Oxnard, CA 93030 named as Additionally Insured
• Must be insured for Ongoing and Completed Operations
• EXAMPLE COI LINK
