Appendix G Data Dictionary Request

This form should be used for initiating new Appendix G research request reviews.

The request will be assessed in terms of the availability of relevant data and the resources required for fulfilling the request.

Please note that you will be required to upload an Appendix G for each requested dataset (please be sure to separate CHARS and Vital Records data) along with the accompanying data dictionary variable list or your submission will be marked as incomplete.

To ensure you have the most current data dictionary, please complete this form. Upon review, we will e-mail you its current variable list and directions for preparing Appendix G documents.

Please indicate which data dictionaries you will need for your study. These dictionaries will be sent to you in a follow-up email once your submission is reviewed.

In addition, we will only provide the fewest data elements necessary for the purpose described in the proposal for research. Please provide a short explanation next to each indicated variable.

If you are selecting multiple data types, please combine into one workbook with multiple sections when you submit them for approval.
