Please be aware that the Request process for the 2023-2024 school year has closed and any requests placed after April 26, 2024 will not be approved by the District Digital Instructional Resource Committee until after the start of the 2024-2025 school year.

Investigated and deteremined that no previously approved digital instructional resource will meet my needs.

Confirmed both my School Administrator and School Tech Contact have reviewed the product requested and provided approval.

If student identifier information needs to be shared with the vendor or imported into an installed program, list the requested data fields. (Verify with vendor information required.)
