Travel Approval Request Form

This form is used to confirm the final pre-departure requirements for all overseas programs. Once you have submitted this form, your response will be assessed and you will be notified of the outcome. This can take up to 3-weeks during peak periods.


Do not submit this form multiple times for the same program.

Monash Abroad uses your application reference number to connect the various forms you submit. Using this number correctly is important for your application's progress.


How to find your reference number:

  • Search your email for "Submission for Application for Overseas Study" or esol.frevvo.notification@monash.edu
  • You'll find the reference number in the subject line and in the body of the email
  • Always copy and paste it directly from the email


Please provide your reference number by copy and pasting. Do not insert any other value than the reference number or your form submissions will not be connected across your application.


Your reference number should be a 36-digit number like this: 3e162c24-eb62-4a8c-9c20-53c7bb4dd4XX

If you are a Monash University Malaysia student participating in:


  1. a short study tour organised by through Monash Malaysia or your school, or
  2. a Monash Malaysia overseas PhD Mobility Program,


your reference number will appear different to the sample above. This does not apply to Monash Malaysia students participating in any other overseas programs.


If you are in one of these two programs, please check your email for a 36-digit reference number beginning with "MUM" and ending with your Monash student ID. If you are unsure of your reference number, please email mum.monashabroad@monash.edu.
