Travel Approval Request Form

This form is used to confirm the final pre-departure requirements for all overseas programs. Once you have submitted this form, your response will be assessed and you will be notified of the outcome. This can take up to 3-weeks during peak periods.

Do not submit this form multiple times for the same program.

Reference number*

Monash Abroad uses your application reference number to connect the various forms you submit. Using this number correctly is important for your application's progress.

How to find your reference number:

Search your email for "Submission for Application for Overseas Study" or esol.frevvo.notification@monash.edu
You'll find the reference number in the subject line and in the body of the email
Always copy and paste it directly from the email

Please provide your reference number by copy and pasting. Do not insert any other value than the reference number or your form submissions will not be connected across your application.

a short study tour organised by through Monash Malaysia or your school, or
a Monash Malaysia overseas PhD Mobility Program,

your reference number will appear different to the sample above. This does not apply to Monash Malaysia students participating in any other overseas programs.

Travel risk and safety

At any point during your Monash overseas program or personal travel will you travel to or transit through a location with a DFAT advisory of "Reconsider your need to travel" (Level 3) and/or "Do not travel" (Level 4).*

Monash Abroad provides online pre-departure training which is a compulsory step as you prepare to travel overseas.

To access the module Monash Abroad Pre-departure Training (MAPDT), click on this link (use CTRL + click to open in new window).

Follow the instructions and complete the training.

For you to satisfy this final requirement submit this form, then proceed to Moodle and complete the pre-departure training in full. You will need to enter your application reference number (see original application confirmation email).

Your approval to travel will be extended only after you complete the MAPDT.
