Lactation Room Access Request Form

Lactation rooms are available for the purposes of nursing or expressing milk only; lactation rooms should not be requested and may not be used for any other purpose. Rooms are available on a first come, first serve basis. Upon completion of the form, you will receive an email from Human Resources within 2 business days confirming your reservation or arranging alternative arrangements. For expedited access to a room due to an urgent need, call 541-346-3159 or email hrinfo@uoregon.edu during normal business hours to receive a temporary code.

Not all lactation rooms are managed by Human Resources. If you don't see your preferred room listed here, go to hr.uoregon.edu/lactation-support-programs for information on how to access the room.

We will reach out at the end of this time period to confirm the end of your access or discuss renewal options.

 
 
 
  • Lactation rooms are available for the purposes of nursing or expressing milk only; lactation rooms should not be requested and may not be used for any other purpose.
  • Users are responsible for cleaning up after themselves and bringing their own personal supplies.
  • Priority is given to those who have scheduled time for lactation support. Please use the door hanger provided to help ensure privacy.
  • When you exit the room, ensure that the door has clicked shut. The locking system is there so users can leave their personal supplies in the room.
  • Do not share your code with others. Anyone who has not been issued a code directly from Human Resources should not be in the space.
  • If you need to change your access schedule, or if you are no longer using the room, contact Human Resources at hrinfo@uoregon.edu.


If you are an intermittent user, please heed the following protocol:

  • Check the room schedule and knock before entering.
  • If the room is occupied, ask if the user is comfortable with you sharing the space for X period of time, politely respect the user’s response.
  • For the comfort of all, if you are dependent on the space, it is important to have your times placed on the schedule. If you are not on the schedule, there is no guarantee that you will be able to access it when needed.
