All clubs with coaching staff should have their coaches complete this form!

If your club practices at Eakin Student Recreation Center, they will need a pass to get in. Once you complete this form we will reach out to your coach directly to provide further instructions on how to access thier pass and provide any training information.

By completing this form I acknowledge that I am an "at will" volunteer with the Club Sports Program at ECU and understand that by nature of my position I will be representing and upholding the values of Campus Recreation & Wellness, and East Carolina University to the best of my ability. Failure to adhere to these standards could result in disciplinary action and/or removal from my leadership position within the club.
