Near-Miss
Knowing about, and addressing, near-miss incidents is a crucial step in reducing injuries. If you witnessed or experienced a near-miss, don’t assume that it was a fluke or that someone else might be taking care of the problem. There’s a good chance that it could happen again to you or someone else, and the next time it may not be a near-miss – it may be an injury. You won’t get in trouble for reporting a near-miss incident. We want to learn about them so we can address the root causes. Remember – our goal is a zero-injury workplace! Please fill out as many details in the form to the right as possible. The form is anonymous, but you have the option to add your contact information at the bottom. Again, you won’t get in trouble for reporting. Including your contact information is helpful if the Safety Committee members have questions they would like to follow up on with you.
