Completion Form - Global Public Health (GPH) Minor/Certificate

Thank you for filing your Summer GPH Minor/Certificate Completion Form!


This form is required in addition to the GPH Declaration Form. This form is required if you wish to be considered for the Summer Sessions Scholarship (Full-Time UC Berkeley students only) and to make verifying completion a smoother process.


Please fill out this form by the priority deadlines of August 1 or February 1. Fill out the form by the priority deadline closest to the end date of your final course.

Scholarship

If you are interested in being eligible/considered for the Summer Sessions scholarship, please fill out the following questions. If you are not, please select N/A and move on.


Please note that the scholarship is only available to declared full-time UC Berkeley undergraduate students. You must be able to answer "Yes" to the following questions to be eligible for the scholarship.

Are you currently enrolled as a full-time UC Berkeley student?*
Have you completed the GPH requirements within the last two consecutive summers?*
Have you completed at least 4 of the 5 courses during the summer?*
Do you have a GPH minor GPA of a 2.0 and all 5 courses were taken for a letter grade?*

The priority deadlines are August 1 and February 1. It is okay to submit this form while you are waiting on final grades.
