The Demobilization form helps the City ensure all business related to an incident is captured accurately. This includes the coming and going of personnel. Completing this form allows the City to adequately plan for ongoing DSW deployment and demobilizations. Click the SUBMIT button when you complete the form.

Identify any technology or supplies you've been using that do not belong to you (or your home Department) and brief your DSW supervisor on what you are leaving behind. (This does not mean communal supplies such as tape and pens etc)

I have cleaned and sanitized my work space and surfaces I have touched recently (If not telecommuting).

DSW TRAINING

All City employees who are actively deployed are required to take baseline FEMA Incident Command System training. If you have not yet completed FEMA Independent Study courses IS 100 and IS 700, follow the steps below: 1. Click on: https://sfgov.org/sfc/employee-gateway and sign in to SF Employee Portal with your login information 2. Click on My Links – Work Links – SF Learning 3. Click on Learning Catalog 4. Using the Search function on the top right, Search for IS 100 or IS 700. 5. Launch course and follow screen prompts to an external link to finish the course. 6. Once complete, follow prompts to mark yourself complete on the SF Employee portal Iink.
