To Redeem Your Prize:

1.Please fill out this form by July 31st, 2025. Forms filled out after this date will not be eligible. Accuracy of information submitted on the Prize Redemption Form is the responsibility of the prize recipient, and incorrect information provided may result in the redemption being invalid.

Please note that some Save a Fortune cards are not prize-winning cards, and only contain fortunes. When claiming a winning card, the prize you select must match the prize on your Save a Fortune card.

2. Between September 1st and November 1st, 2025, please visit the Canna Cabana location you have selected and present both your winning scratch card and your ID to your budtender. Please note that prizes must be picked up by November 8th, or they will be deemed invalid.

For papers or cones from Canadian Lumber and Zigzag, batteries from Good Supply, Tribal, and Cookies, doob tubes from HMP, lighters from clipper or keychains from Cabana Cannabis Co, there is no need to fill out this form. Simply bring your card to your budtender to receive your prize anytime before November 1st, 2025!
