If medical treatment is needed, the Stable will attempt to contact the Owner, but in the event the Owner is not reached, the Stable has the authority to secure emergency veterinary and/or blacksmith care. The Owner is responsible for paying all costs relating to this care. The Stable is authorized as the owner’s agent to arrange billing to the Owner.

While the horse is boarded at Adena Springs North, Adena Springs, and Ontario Equestrian shall not be liable for any sickness, disease, theft, death, or injury suffered by the horse(s) or any other cause of action arising from or connecting to the boarding of this (these) horse(s). The Owner assumes all risks. All costs, no matter how catastrophic, connected with boarding are borne by the Owner. The horse(s) and all tack and equipment of the Owner lodged on the Stable premises shall be at the sole risk of the Owner, who assumes all responsibility for ensuring the horse(s), tack, and equipment owned by him/her, while on the Stable premises.

Please indicate what time you plan on shipping in on Thursday, October 5th. We understand that this is subject to change closer to the date.

Horses must ship out on Sunday, October 8th. The camp will conclude at approximately 3 p.m.

# of Bags of additional shavings requested*

Each horse will receive 4 bags of complimentary shavings and a box stall in Barn 8. Other items needed for care are the responsibility of the athlete. Additional shavings are $9.00 per bag.

i.e Feed, Water Buckets, Hay, etc.

Coggins test within 6 months
Proof of up-to-date vaccinations
A Statement of Health must be signed by a licensed veterinarian within 30 days of arrival at NW Stables LP, documenting that the horse has:

Received regular and consistent vaccination against Equine Influenza and EHV-1/4 with the most recent booster being within 180 days (as per USEF GR 845) and not within 7 days.
Not shown symptoms of or been treated for EHV-1/4 within the past 28 days.
Not been at a facility under quarantine for EHV-1/4 or exposed to any horses showing symptoms of EHV-1/4 within the past 28 days.
Current vaccination against WNV, EEE, WEE, Tetanus, and Rabies.

AGREEMENT FOR ACCEPTANCE OF RISK*

I request permission to board my horse and participate in horseback riding and other activities at NW Stables, LP, 14875 Bayview Avenue, Aurora, Ontario, L4G 0K8.
I fully understand that horseback riding, handling and grooming of horses, and other stable activities can be dangerous.
I wish to participate in these activities knowing that they are dangerous.
I accept and assume all the risks of injury (including death) to me, my property, or anyone I bring onto the property.
I have read and understand the safety laws regarding helmets and appropriate footwear and understand that riders under the age of 18 must at all times wear an approved riding helmet at any and all times while riding.
I agree that while riding on or off the property of the Stable appropriate care should be taken at all times to ensure that the areas are clear of debris, prior to taking any actions that may cause injury to myself or my horse and to use caution at all times.
I agree and understand that at no time can the Stable or OE be held responsible for any damages, illnesses, or injury leading to medical care of death for either myself or my horse.
