PacNW Work Program Application

Pacific Northwest Soccer Club (PacNW) has many events that rely on volunteers for success. When the volunteer slots are not filled, PacNW may hire members to work an event for pay.

The PacNW Volunteer Program and PacNW Work Program are separate programs and can NOT be interchanged. PacNW does not discriminate on the basis of race, color, religion, national origin, sex, age or disability.

The family volunteer requirement must be completed prior to enrolling in the PacNW Work Program.

Requirements for Work Program Applicants: *

Must be 14 years of age or older *
Must submit a completed and signed IRS W-9 form https://www.irs.gov/pub/irs-pdf/fw9.pdf *
Applicants age 14-17 must complete and submit the Parent/School Authorization form by Sept 30th or prior to the event worked. (Must be signed by both the parent and the school).

The Parent/School Authorization form specific to PacNW Work Program can be found by clicking on the "Parent- School Authorization Form" button on the PacNW "Forms/Documents" webpage and select the form for "WORK STUDY" https://www.pacificnorthwestsoccerclub.org/resources/forms-documents/

In addition, applicants age 14-17 must upload a completed, PacNW specific, Parent School Authorization form. This form can be found on our webstie by clicking on the parent-school authorization form button at https://www.pacificnorthwestsoccerclub.org/resources/forms-documents/ This form must be completed and signed by BOTH the parent AND the student's school.
