500P - Pasteur Staff Garage (PSG) Badge Access Request Form

Please fill out this form to request night shift or weekend badge access to the Pasteur Staff Garage. Please ensure the guidelines below are followed.


1. Entry is permitted on weekdays between 4PM and 4AM and 24 hours on weekends and holidays. A grace period for exiting has been added to allow exit with badge access until 7AM.


2. Staff MUST leave the Pasteur Staff Garage before 7AM, Monday -Friday. If staff leaves after 7AM on weekdays, Monday-Friday, a parking fee of $12.00 will be applied upon exiting.

I understand that if I leave the Pasteur Staff Garage after 7AM, I will be subject to the daily parking fee. I understand that overnight parking is available at the Pasteur Visitor Garage located at 200 Pasteur Drive.

I understand that Transportation Services conducts routine reviews of badge activity to remove unneeded access, and that my badge access may lapse or be removed if not used within a 6 month period. If my badge access lapses, I understand that I will be required to re-apply to regain access, and that I must meet all eligibility requirements for access at that time.

Parking at Stanford Medicine lots and garages is at your own risk. Please remove valuables before leaving the vehicle; Stanford Medicine is not liable for lost or stolen items. Should you incur damages or losses, they must be reported before the vehicle is retrieved from the facility.
