Math Performance Success (MPS) Application

Thank you for your interest in the MPS program! Before proceeding with your application, please note the following important details:

•    Course Enrollment: You must be enrolled in both the math course and its corresponding supplemental support class with the same instructor.

•    Commitment: By applying, you are agreeing to the following:

  1. Attend an MPS orientation session.
  2. MPS orientations will be held via Zoom every Thursday from 4:00 PM - 4:30 PM, starting from the first Thursday in June through the end of June.
  3. If you cannot attend in June, additional orientations will be available on the following dates from 4:00PM - 4:30PM via Zoom:
  • Thursday, July 18
  • Thursday, July 24
  • Thursday, July 31
  • Thursday, August 4
  1. Regularly attend all classes. Classes meet four days a week: Monday, Tuesday, Wednesday, and Thursday from 10:00 AM to 12:15 PM. (A maximum of 5 absences is allowed per quarter).
  2. Participate in academic support services such as tutoring, workshops, and individual academic counseling.
  3. Stay on track with homework, lab assignments, and class activities.
