Contractors are not permitted to share promotional materials with clients without prior approval from the Senior Center Manager. Please upload any promotional materials you would like reviewed.

Will information be collected from Senior Center participants?*

Please provide a detailed description of the specific information that will be collected from senior center participants, as well as a description of how that information will be used.

Will you be charging a fee for this service?*

Please describe the pricing for the proposed service. Pricing must include either a total cost (maximum cost per service) or a cost per person, depending on the proposed service. Pricing must include all applicable costs and fees. If selected to provide an approved service, the Service Provider will not be permitted to charge any fees beyond those described below.

  2. Background Checks (one current background check is required for each individual who will be providing service)
  3. General Liability Insurance (please refer to the Sample Senior Center Contractor Agreement for details on insurance requirements)
  4. Presentation/Class/Exhibition Materials (for presentations, classes, or exhibitions, you must upload a copy of any audio/visual presentation, as well as all educational materials that you want provide to participants)
  5. Promotional Materials (in general, AAS does not allow Service Providers to share promotional materials with participants; if you would like have your promotional materials reviewed by the selection committee, please upload them here)
  6. Professional Certifications (copies of any relevant professional certifications)
