Wests Future Stars Engagement Form

The purpose of this engagement form is to Provide a monthly update on the activities, progress participation and attendance, completed by the athlete as a Wests Future Star, including;


  • Performance updates for Program Coordinators to interact with athletes to provide support within the program, and primarily in areas of Load Management, Athlete Support, Leadership and Mentoring.
  • Performance information and images that can be used in media releases / social media stories to promote the athlete and the Wests Future Stars program
  • Additional feedback on topics / education for inclusion within the program.


This form is due to be completed on the 1st day of each month, whilst a part of the Academy Wests Future Stars Program.


Athletes are reminded that they should be maintaining a form of Load Management Diary (or use of the Athlete Journal / XPS) to record information during the month in the lead up to completion of this form.


And are encouraged to reach out to sport@swsas.org.au at any time should they wish to discuss aspects of the program with either Academy staff, or Wests Future Stars Program Coordinators.

Goal Setting and Individual Athlete Performance Plans

Load Management

Please rank your past four (4) weeks in terms of Load.


Include Average hours spent each week training or competing in competition for each intensity level.


ie: Easy - 4 hours, Moderate - 1 hour, Hard - 7 hours.


Key:

Easy - Physically / Mental Low Intensity

Moderate - Reasonable physical / Mental Intensity

Hard - Physically / Mentally High Intensity (Demanding)

Include events, training sessions for Academy, Home Club, School, State commitments etc; including on-field/court and off-field commitments (ie school work, assignments etc)

Including Strength and Conditioning programming, changes or proposed changes to the program , and feedback on your S&C program goals and outcomes.
