Have you (or your research team) made any changes to the research project since it was approved by the IRB? The IRB also asks that you provide written explanation for any changes. It should be attached with this form but can be sent afterwards directly to irb@nmt.edu.

 

A brief project progress report is required before the renewal will be reviewed. It should be attached with this form but can be sent afterwards directly to irb@nmt.edu.

Is a faculty advisor assurance required?

 

 

Multi-year projects are initially approved by the IRB for a 12-month period. Please attach a brief report describing your progress to the IRB to this form. Attach any other necessary documents here as well.
