Earth Day Participant Release of Liability & Photo Consent

Thank you for registering for Mason CD's Earth Day Celebration at

Huff 'n' Puff Trails. We are thrilled to partner with community members like you to help restore this beautiful green space.

Huff 'n' Puff Trails in located in uptown Shelton, across from the Shelton High School. It is owned and managed by the City of Shelton. It has 5 different trail loops that can be taken and add up to roughly 1.8 miles of trail. Native fauna and flora of the Pacific Northwest can be found throughout such as black-tailed deer, Douglas squirrels, Gary oaks, and evergreen huckleberry.

Local recreational areas, such as Huff 'n' Puff Trails, play a vital role in supporting the health and well-being of the community, while also contributing to the environment.

Below, you’ll find the waiver of liability form, photo consent form, and a brief survey. We kindly ask that you complete each of these forms.

The Undersigned, on behalf of themselves and their estate, at this moment waives any right of recovery and releases the Mason Conservation District (DISTRICT) and the City of Shelton from liability related to the Undersigned, arising from any injury to persons or property damage. The Undersigned further agrees to indemnify, hold harmless, and defend the DISTRICT and City of Shelton from and against any claims, damages, actions, liability, and expenses, including attorney’s fees and other professional fees, in connection with bodily injury, including death, personal injury, and property damage arising from or out of the Undersigned’s activities and participation in volunteer services at the above project site.

The Undersigned further acknowledges and agrees that neither the DISTRICT nor City of Shelton assumes any responsibility whatsoever for any property of the Undersigned, and the Undersigned shall not hold the DISTRICT or City of Shelton liable for any loss or damage to such property.

For volunteers under the age of 18, this form must be signed by the parent or legal guardian of the minor volunteer participating in the project.

I give my permission for myself and my child/children herein named, understanding that the pictures may be used for Mason Conservation District (DISTRICT) and City of Shelton publications and related publicity. I understand that neither my child/children nor I shall receive any compensation for such use of our photographs.
