TECH & AV EQUIPMENT LENDING AGREEMENT

When borrowing an iPad, Laptop, or audiovisual equipment from the Library I understand and agree to the following:


1. I accept full financial responsibility for all items and accessories while they are checked out to me.


2. I understand that if I do not return the iPad, Laptop, or A/V equipment on the assigned due date, I will be charged $20/day (up to $100). At this point, the device will go to lost status and my library account will be blocked. Once the item is lost, I will be charged and held financially responsible for the full replacement cost until the item is returned. Other fines, fees, and holds on my student account may apply if this item stays lost, or I fail to communicate with library staff. I will be held financially responsible for repair or replacement. I understand that I will be held liable for any missing or damaged components.


3. I will not loan iPad, laptop, or equipment borrowed to anyone else, irrespective of his or her relationship with Northwestern University.


4. I will immediately report any loss, damage, or malfunction of iPad, laptop, equipment or accessories to Library staff.


5. I will return all devices and their accompanying accessories at the same time. If travelling with the device during the loan period, it is still due back on the designated date. All items must be returned to the Main Library Circulation desk during hours when Circulation staff is present. Do not return any technology items inside any of the book drops or leave them with other service points in the building.


6. I will not tamper with hardware or software. I will not remove Library markings or barcodes, deface or affix anything to the device.


7. I understand that Northwestern University Library is not responsible for any files or personal information left on returned materials. All data accessed or stored on the iPads and laptops will be erased upon its return to the Library. I am responsible for deleting content stored on AV equipment.


By signing below, I indicate that I have read, understand, and accept all of the terms and conditions stated above.


Items will remain on patron Library accounts until processing has been completed.


Please note: All devices returned after 5pm M-TH will be processed by Library staff on the following business day. Devices returned over the weekend will be handled on Monday.
