2018 ASHA Convention Function Space Request Form
ASHA typically holds extra space to support affiliate events. Space will be assigned on a first-come first-served basis as it is available. We are usually able to schedule all requests, however, we have had an increasing number of requests each year. If we are unable to accommodate a request we will help facilitate alternate arrangements with one of our partner properties.
Scheduling of activities during educational programming will be limited to groups of 25 people or fewer. Please consult the Schedule at a Glance and plan your events before and after Convention programming, generally before 8:00 AM and after 6:00 PM Thursday through Saturday.
Complete one function space request form for each activity per day and/or non-contiguous time block.
All companies or organizations will be responsible for the cost of any food and beverage, audiovisual equipment, room set changes, or additional specifications requested and will be billed directly by the facility at which their event is scheduled.
The deadline for request submissions is August 1, 2018. Notifications of location and room assignments, along with contact information for arranging audiovisual items and food & beverage, will be sent via email no later than October 1.
Please be brief. Titles are limited to maximum of 10 words.
We suggest spelling out acronyms as this helps us identify your event when asked by attendees looking for it onsite.
Most events are held between Wednesday and Saturday (Nov. 14-17). Function space availability on Tuesday is extremely limited. Open House night is Friday, Nov. 16 following the ASHA Awards Ceremony.

Indicate whether this event should appear in publications such as the ASHA Leader, Web site or daily list of activities distributed onsite at the convention.

What time will your event start? This is the time that will be printed if you have indicated that you would like your event published.

Access to the room will be allowed 30 minutes prior to your event's start time unless you request an earlier set up time in the Additional Information section at the bottom of this form.

Provide your best estimate of how many people will attend this event so we can assign an appropriately sized space.
If Other, please specify in Additional Information. We will make every effort to accommodate your desired room arrangement, however, note that room set changes may incur fees.
Round Tables (10 chairs/table)
Will your event require any audiovisual equipment? Your response will assist us with scheduling your event.
If you indicate yes, we will provide contact information to arrange the equipment you need with your room assignment.
Please note that even if you indicate that you don't need any A/V equipment, you may be be scheduled in a room that already has equipment present (you will not be charged for a prior or following event).
Will your event require any food and beverage or catering? Your response will assist us with scheduling your event.
If you indicate yes, we will provide contact information to arrange catering with your room assignment.
