I/We acknowledge that any event or activity that includes photography must display the photo permission sign prominently at the front entrance of all events.

Please note that if the event involves patient care—such as a community vaccination clinic in a public location—this sign will not fulfill legal HIPAA requirements. As usual in inpatient care settings, people under our medical care must sign a HIPAA consent form before having their picture taken. Please email hs-marketing@ucdavis.edu for copies of the HIPPA consent form.
