Use this form to request paper test books to be sent to your school for student(s) that require them due to an accommodation in their plan. If you have multiple students that require paper test books, submit this form for each student. This form must be filled out at least 3 weeks prior to your scheduled test date.

Additional Accommodations

This section will ask about other additional accommodations that the student testing with paper may require. This will help us determine which type of accommodated materials to send to you. For example, a student requires the paper test book as well as breaks as needed and double time. You should enter breaks as needed and double time below. NOTE: All accommodations must also be requested and approved in SSD Online prior to administering the assessment.

Also include English Learners using time and one-half extended time. If the student does not have any additional accommodations and is only using the paper test book, include "None".

Please indicate any state-allowed accommodations that the student is approved for in SSD Online. These accommodations do not result in college or scholarship reportable scores and should be used in rare instances.

Please read the following points and confirm that you understand.

I understand that: 1. I will not submit a request for a paper test book accommodation in SSD Online. 2. College Board will only ship me accommodated materials based on the accommodations I indicated on this form. 3. I must request approval for any additional accommodations in SSD Online in order for the student to use them on College Board assessments. 4. The student may not use the additional accommodations I indicated above unless designated as approved in SSD Online. 5. Paper test materials will be delivered to my school approximately 1-2 weeks prior to test day. 6. I will refer to the Nonstandard Administration Report (NAR) in SSD Online to determine what color test book my student will use and whether they are allowed to test in the accommodated testing window or if they must test on the primary test date. 7. I understand that if students test with a purple test book they must complete the test in 1 day and on the primary test date for my state.
