2025 Affiliate Monthly Ministry Report

1) This form should be filled out monthly by the Ministry Director or a designated leader so that numbers are only counted once for any project or ministry. 2) When a number is asked for, please use a numeric answer, a number, for example: 1, 25, 250, etc. DO NOT USE WRITTEN ANSWERS WHEN NUMBERS ARE ASKED FOR, for example: two, thirty, seventy five, etc. ONLY USE NUMBERS. Not words for numbers. 3) The numbers asked for are for the current month unless otherwise stated. A few items ask for weekly numbers. NO YEARLY NUMBERS should be reported on this form. Yearly numbers go on the Affiliate ANNUAL Ministry Report.

Where you live and minister

Please report the amount of income you received from GTN this month and your main two ministry related expenses.

Please Note that combined totals for those you ministered to in all settings, each of the following categories of ministry, will be asked for at the end of this ministry section.

Read through the fields to determine the correct category to report your ministry activities and engagement.

Please use this section if you and your team organize relief efforts for those affected by disasters.

This section is for meetings you lead or held, personal or corporate, in which you preached, taught, shared, etc., in an effort to engage the target audience, be they believers or non-believers, in a church setting, an outreach setting, one-on-one evangelism, etc. to make a decision for Christ, to be active in their faith, in missions, engage the Great Commission, work together to advance the Kingdom, and so on. THESE ARE NOT ONGOING LEADERSHIP AND DISCIPLESHIP PROGRAMS, or Go To Nations or Affiliate Bible School or Church Empowerment Programs. There are separate fields to report those. These are one time meetings for evangelism or outreach, single day or several day meetings, Church Services, Conferences, Prayer Breakfast, Crusades, Outreach Events, etc, where you were a presenter, speaker, evangelist, teacher, etc. or the primary speaker and endeavored to engage and activate those attending for evangelistic purposes, or for greater ministry and/or missions involvement.

These should be ongoing Leadership Development or Discipleship meetings you have with an established group or individuals that are consistent and planned to bring leadership and discipleship growth over time. (Not just a meeting or single encounter, but regular meetings over time promoting growth.)

*These may or may not use a defined curriculum. They may or may not have the possibility of graduation or completion of a specific program over time. But are led by a GTN member for/to or with non-GTN members as the primary attendees to see them grow in leadership and discipleship over time.

Churches, House and/or Cell Groups are included in this section.

This is for ongoing projects focused on lasting change and transformation in lives and therefore communities. There are fields for two ongoing projects or programs in a given month.

At Risk are those that are marginalized, at risk of severe poverty, living in severely unhealthy environments, have little hope of education, and may be exploited by others.

This section is for Various types of media presentations and engagement. DO NOT INCLUDE these numbers anywhere else.
