Lean Six Sigma Green Belt Scholarship Application Form
As part of the Provost's initiative to build a Culture of Continuous Improvement at UC Davis, funding is allocated to invest in Lean Six Sigma Green Belt training. This training is designed for individuals who will undertake a process improvement project resulting in a minimum of $10,000 in either hard or soft savings for UC Davis.
Please complete the application below, ensuring all required fields are filled out. Each applicant must submit their own application, even if they intend to collaborate on a project.
Scholarships will cover the full course fee of $2295. To apply for a scholarship, approval from your unit's Assistant Dean, Chief Operating Officer, or equivalent leadership is required. Once the application is submitted, it will route to the identified leader for approval.
Final day to apply for scholarship is Wednesday, December 18, decisions will be communicated to applicants the week of January 13, 2025. More information available here.
