Nationwide Children’s Hospital School-Based Health Center Appointments! (Reynoldsburg)

If you are interested in scheduling an appointment for your child(ren) at Nationwide Children's Hospital’s School-Based Health Center, please complete this form. A hospital staff member will be in touch with more details.

We offer Primary Care Services including checkups, sick visits, physicals,school-required vaccines and more. We can also diagnose and treat anxiety, depression and ADHD. We also offer vision services including eye exams, glasses adjustments, as well as assistance with ordering new glasses.

Please submit one form per child. The health center is located at the Reynoldsburg High School Livingston Campus and is open to all children in the community, ages 0-21. We accept Medicaid and other forms of insurance. Financial assistance is also available.
