Pre-accreditation criteria for Community Quality of Care Centres (QCCs)
DEFINITIONS
ICARe-HF: Improving Care through Accreditation and Recognition in Heart Failure
Community Quality of Care Centres (QCCs) include primary care institutions, resident cardiologists, and local (community-based) hospitals and rehabilitation centres. Their role is to provide initial assessment and treatment of patients with mild acute/decompensated HF and to ensure the optimization of therapy in chronic HF.
QCC Director: the QCC director is the director of the centre/institution/HF unit
QCC Coordinator: the QCC Coordinator is the local HF expert within the centre/institution/HF unit
The QCC Director and the QCC Coordinator can be the same person.
