YMCA Association-Wide STAFF COVID-19 Screening Form

As of March 21, 2022, staff are no longer required to complete the YMCA Association-Wide Staff COVID-19 screening form prior to entering a YMCA worksite. Individuals are expected to “self-screen” (monitor themselves for symptoms and refrain from entering our centres and offices when required to isolate/quarantine by Public Health directives) before entering any YMCA location. Furthermore, screening results no longer have to be confirmed.


If your program area requires you to continue with daily screening, please speak with your leader for more details about the required process.


Staff who wish to continue using a screening tool to help them make decisions about whether to come to work or not can use the Ontario COVID-19 worker and employee screening tool.
