All research activity associated with UCSF must comply with academic and governmental standards regarding the ethical principles and guidelines for the conduct of research involving human or non-human subjects. · All geographical subdivisions smaller than a State, including street address, city, county, precinct, zip code
· Dates directly related to an individual including birth date, admission date, discharge date
· Phone numbers, fax numbers, email addresses
· Social Security numbers, medical record numbers, account numbers, Certificate/license numbers, vehicle identifiers and serial numbers, license plate numbers
