This form lets us know more about the games you are bringing to PAX and if any special conditions are required to display them. If you have any games or demos that will be unclassified at the time of PAX, you must let us know by completing this form (one for each game).

If your games will all be classified, you will only need to complete this form once, though you are responsible for ensuring your games are all displayed in accordance with the appropriate advertising guidelines.

You are only required to complete this form if you are showing a video game or video.
