June Vascular Access Monthly Report -- Track 1 (DaVita Focus Group)

This month, the Network wants to know what you learned from the Patient Scans we asked to be completed in June and how the feedback from these Scans assisted your facility in your quality improvement plan (PDSA) to reduce long term dialysis catheters.

The Vascular Access Experiences (Patient Scans) can be found on the Heartland Kidney Network website - Dialysis Catheter Reduction through Patient and Family Engagement Interventions - 2017

The number of patients using a catheter as their vascular access (for 90 or more days) as of the patient's last hemodialysis treatment of June.

Total number of hemodialysis patients receiving treatment at the dialysis facility as of the last hemodialysis treatment of June (total facility hemodialysis patient census).

You had the choice of using one (or more) of the four Vascular Access Experiences (Patient Scans) in June. Enter the total number of scans completed.

Understanding the patient's perspective in regards to their vascular access experiences adds knowledge to your quality improvement plans. The Network would like to know the usefulness of the scans provided to you. (Choose one)

The Network would like to know the main venue you used to share the results of your patient scans and how you incorporated the feedback in your PDSA cycle.
